This work synthesises the scientific literature on Myalgic Encephalomyelitis / Chronic Fatigue Syndrome with structured epistemic rigour — separating what is established from what is hypothesised, speculated, or unknown. Every claim is classified as an achievement, hypothesis, speculation, prediction, warning, or recommendation, with an explicit certainty rating.
2 Motivation
The medical literature on ME/CFS is scattered across thousands of papers, often contradictory, and rarely synthesised in a way that serves both clinicians and patients. This documentation exists to bridge that gap — to make the evidence accessible, structured, and honest about its limits.
3 Methodology
The paper uses a multi-layer approach:
- Structured epistemic classification — every statement tagged by evidence level
- AI-assisted literature synthesis — with mandatory human critical oversight
- Adversarial review — multiple AI personas attack the content from hostile angles
- Formal modeling — ordinary differential equations for disease dynamics
- Open source — all content, code, and build tooling publicly available
