Emerging and Investigational Therapies
This chapter surveys emerging and investigational therapeutic approaches for ME/CFS identified through novel immunophenotyping, cross-disease mechanistic inference, and targeted pathway intervention. None of the therapies discussed have completed randomised controlled trials for ME/CFS; they are presented as mechanistically motivated research directions with explicit certainty ratings rather than clinical recommendations. Patient safety, particularly for severe and very-severe patients, and the need for rigorous clinical trial validation are emphasised throughout.
For patients: the only immediately actionable item is the emergency PEM prevention protocol (Emergency PEM Prevention Protocol) — but it is speculative and unvalidated; discuss with your physician first. Everything else is research-stage.
For caregivers: the PEM prevention protocol may help you prepare a crash response, but treat it as a hypothesis, not an established protocol, for a severe patient.
