ICD Classification and Disability Recognition
ME/CFS has an ICD-10 code (G93.3, “Postviral fatigue syndrome / Benign myalgic encephalomyelitis”) and an ICD-11 code (8E49, “Postviral fatigue syndrome”). The existence of a code is a necessary condition for disability recognition — but it is not sufficient. Podell et al. detail the substantial documentation burden required to convert an ICD code into a successful disability claim even in the United States, where Social Security Disability Insurance (SSDI) provides a formal adjudication framework (Podell2020documentingDisability?).
In LMICs where disability insurance is weak or absent, ICD coding confers no practical benefit. A patient in Nigeria with ME/CFS carries the same diagnosis as a patient in Oslo — but without access to disability benefits, workplace accommodations, or specialist medical care, the diagnostic label alone changes nothing about their functional reality.

Consequence: The WHO’s ICD-11 expansion to include ME/CFS as a distinct entity was a global-level policy win. But policy wins at the global level do not translate to protection at the local level unless national disability systems adopt and operationalize the code. For most LMIC patients, ICD-11 8E49 is a label on paper that no local doctor will assign and no local disability office will recognize. Severity applicability: all — the recognition gap affects all severity levels, but the functional impact is most severe for the severely disabled, who have the greatest need for disability support.
Falsifiable prediction: If a systematic audit of ≥10 LMIC national health information systems finds ≥3 where ICD-11 8E49 has been adopted AND appears in ≥1% of primary care encounter records AND is linked to a disability benefit pathway, the ICD-code-as-empty-vessel claim is overstated — the code has practical effect in some settings. If zero countries meet all three conditions, the empty-vessel hypothesis is supported.
