Exercise Meta-Analyses: The 2025–2026 Cluster
1 Zhao et al. 2026 — Exercise in ME/CFS: Subjective Improvements, Null Objective Outcomes
Full Citation:: Zhao L, Gou B, Zhang M. The efficacy of exercise in patients with myalgic encephalomyelitis/chronic fatigue syndrome: A systematic review and meta-analysis. Journal of Psychosomatic Research. 2026. (Zhao, Gou, and Zhang 2026) DOI:: 10.1016/j.jpsychores.2026.112136 Key Findings::
- 17 RCTs, n=1,944; RoB 2.0 found "prevalent high risk of bias across the included studies"
- Exercise improves fatigue (SMD=0.85), sleep, QoL, depression, anxiety — all subjective
- No improvement in cardiopulmonary function or objective functional capacity
- Mixed diagnostic criteria: Oxford, Fukuda, Reeves — none requiring PEM
- Pooled aerobic exercise with yoga, qigong, tai chi
Critical Interpretation:: The subjective-only improvement with null objective results is the predicted signature of blinding bias per Savović 2012 and Hróbjartsson 2014. The “physiological ceiling” explanation is less parsimonious than the bias explanation. Certainty Assessment::
- *Quality:* Medium — acknowledged high RoB throughout; no PEM-criteria sensitivity analysis
- *Replication:* Pattern replicated in Wei 2025 and Liao 2025 (same structural flaws)

Full Citation:: Vink M, Vink-Niese A. The Updated NICE Guidance Exposed the Serious Flaws in CBT and Graded Exercise Therapy Trials for ME/CFS. Healthcare. 2022;10(5):898. (Vink and Vink-Niese 2022) DOI:: 10.3390/healthcare10050898 Key Findings::
- PACE trial: 20% of participants who improved subjectively had deteriorated objectively
- Oxford criteria inflate efficacy by including non-ME/CFS patients
- Cites Savović 2012: lack of blinding → 13% average exaggeration
Relevance:: Bridge paper connecting PACE-era critique to the BRANDO blinding framework. Certainty Assessment::
- *Quality:* Medium-high — *Healthcare* (MDPI); peer-reviewed
- *Replication:* NICE 2021 guideline change independently corroborates the critique
