- 17 RCTs, n=1,944; RoB 2.0 found "prevalent high risk of bias across the included studies"
- Exercise improves fatigue (SMD=0.85), sleep, QoL, depression, anxiety — all subjective
- No improvement in cardiopulmonary function or objective functional capacity
- Mixed diagnostic criteria: Oxford, Fukuda, Reeves — none requiring PEM
- Pooled aerobic exercise with yoga, qigong, tai chi

Critical Interpretation:: The subjective-only improvement with null objective results is the predicted signature of blinding bias per Savović 2012 and Hróbjartsson 2014. The “physiological ceiling” explanation is less parsimonious than the bias explanation. Certainty Assessment::

- *Quality:* Medium — acknowledged high RoB throughout; no PEM-criteria sensitivity analysis
- *Replication:* Pattern replicated in Wei 2025 and Liao 2025 (same structural flaws)

2 Vink & Vink-Niese 2022 — NICE Reversal Exposes GET/CBT Trial Flaws
