Mechanistic and Experimental Studies
This chapter reviews the experimental and mechanistic studies that probe the biological basis of ME/CFS. It opens with the 2024 NIH deep phenotyping study and related investigations, then surveys the evidence across the domains in which the disease is best documented: exercise physiology and post-exertional malaise, cellular and molecular findings, neuroimaging, immunology, and multi-omics. It also covers emerging lines of inquiry, including GPCR autoantibody research and integrative multi-omics approaches. By aggregating mechanistic findings from independent laboratories, the chapter assesses the strength and convergence of the evidence that ME/CFS is a systemic biological disease rather than a psychological state.
